Office Policies
FAQ

We are pleased that you have chosen us to support you and your family in your health journey.

The following is a list of our most common office policies and procedures for your review and convenience to help our office run as smoothly as possible for both the providers and the patients:

Appointments: 

  • We value the time we have set aside to see and treat you or your child.  If you are not able to keep an appointment, we would appreciate a minimum of 24-hour (business hours) notice.  There is a charge of $50 for missed, pre-scheduled appointments.  Prices are subject to change.

  • Please arrive on time for your appointment. Our time is reserved exclusively for your care during your scheduled visit.  If you are more than 10 minutes late, we will do our best to accommodate you. We may need to shorten the appointment and still end at the scheduled time to accommodate the next patient. However, on certain days, it may be necessary to reschedule your appointment entirely.

  • We strive to minimize any wait time; however, emergencies do occur and will take priority over a scheduled visit.  We appreciate your understanding.

Transfer of Records:

  • If you transfer to another physician, we will provide one copy of your most recent medical records, free of charge.  We can fax it to the physician’s office.  It may take up to 30 days to process your record request. 

  • We can only provide records of your child’s visits rendered here at Thrive Center for Health only.  For any other previous records, you must request directly from your previous doctor(s).

  • If additional records are being requested by the parent, the cost is $0.65 per page.

Prescription Refills:

  • For medication refills, we require 48-72 hours’ notice, during regular business hours.  Please plan accordingly.  These can be requested by email or by phone.

Financial Responsibility:

  • If we do not participate in your insurance plan, payment in full is expected at the time of your visit, unless other arrangements have been made in advance.

  • This includes patients who have UnitedHealthcare as we are no longer in-network. 

  • We bill most major insurance carriers as a courtesy.  It is ultimately the patient's responsibility to ensure payment for the services rendered are made to the provider in a timely manner. 

  • If previous arrangements have not been made with our office, any account balance outstanding longer than 30 days from the date billed will be subject to dismissal from the practice.

  • For scheduled appointments, prior balances must be paid before the next visit.

  • A $25 fee will be charged for any checks returned for insufficient funds.

  • A $10 late fee per month will be applied to outstanding balances.

Membership Fee:

  • Due no later than February 15th of each calendar year, you are responsible for the annual membership fee if you choose to continue primary care at Thrive. 

  • Pricing:

    • One family member: $100/year

    • Two family members: $140/year

    • Three family members: $180/year

    • Four or more family members: capped at $220/year

  • This fee specifically covers services not covered by your medical insurance within the integrative framework. 

  • If you do not pay the fee by February 15th, you will be put into inactive status, and will not be able to schedule appointments until the fee is reconciled. 

  • This fee is non-refundable.

Insurance:

  • It is your responsibility to understand your benefit plan with regard to covered services and participating laboratories.

  • When a  baby is born, please provide us with the insurance information within 30 days

  • It is your responsibility to know if a written referral or authorization is required to see specialists, whether preauthorization is required prior to a procedure, and what services are covered.

  • It is your responsibility to notify us if your insurance carrier or any of your demographics change, in order to prevent any unnecessary delays and duplicated work for the staff.  If this is not done prior to your appointment, there is a $25 fee.

Referrals:

  • Advanced notice is needed for all non-emergent referrals, typically 3-5 business days.

  • It is your responsibility to know if a selected specialist participates in your plan.

  • If a referral is requested by the parent/ patient (as opposed to the provider), it may incur a fee of $20

Pictures and Videos:

  • We ask that you do not send pictures or videos unless you are requested to do so.  You will need to make an appointment to bring your child in to be seen if you would like something evaluated and/or diagnosed.

School/Daycare Forms:

  • Please allow 5 business days for forms to be completed.

  • Parents need to supply the required forms to the office. Please fill out as much information about your child as you can. 

  • If you would like us to mail the completed form back to you, please provide a stamped, self – addressed envelope. Otherwise we can fax or email it to your requested contact. 

  • If you are requesting a form outside of your appointment time, there is a fee of $15 per child. 

 

Urgent/ Same-day care:

  • Our providers reserve time in their day for advice and same day urgent office appointments. This allows a more extensive intake, the ability to order labs if necessary and to discuss a treatment plan.

  • The earlier in the day you are able to call will increase our likelihood of accommodation.

  • Please remember the front desk is only able to answer administrative questions.

Urgent After Hours advice:

  • Our providers are available for urgent after hours calls for primary care patients that cannot wait until the next business day.

  • Typically if you need to be evaluated when the office is closed it is best to go to urgent care of the ER. We do not use a nurse answering service, one of our providers will be responding to your call.  Please be mindful of the reason for the call and if it could wait until the next business day.

  • After the office closes we are available until 9pm, and then call resumes the following morning at 7am.

  • The providers are not reimbursed for these types of calls. If the complexity warrants a visit, it will be recommended that your child be seen at an Urgent Care, ER, or to call the office for an appointment during business hours
